Take a self portrait photo and convert it into black and white, as below:

Then, remove the background, filling it with plain black.
Using the "liquify" tool, drag the bottom jaw down and push the sides of the jaw in slightly.
Creat a new layer and use a "darken" brush with a low opacity to darken the shadows. Follow this with a "lighten" brush to accentuate the highlights and give the image more contrast and depth.
Working on the eyes, remove the pupils with a plain white brush and replace them with a solid black dot. Add catchlights to the eyes.
For an added touch, go back to the "liquify" tool and push some of the teeth into a point.
Continue to work on the shadows and highlights until you are happy with the final results, see below:
